Marriage Prep Class Registration Form

This form is to be filled out by the engaged couple at least two (2) weeks prior to the retreat date. Or click here for a PDF version of the form and mail to the parish office at 411 Vandenbroek Street, Little Chute, WI 54140.

There is a $75 registration fee for this class. We accept a check (made payable to St. John Parish), cash or Venmo.

The Marriage Preparation is held at St. John Nepomucene Catholic Church in the Fellowship Hall and in the Msgr. Jim Room. The day will begin at 8:30 a.m. and will end at 5:30 p.m. This includes a continental breakfast and lunch as well.
